This is apparently a re-issue with the Index. Two leaves of ‘Nova statuta’ by Hugo de Loubenx Verdala have been interpolated between p. 198 and p. 199 (the map.) In the preliminaries, 3 leaves have been added; a dedication by Joannes Baptista Rindinellus who seems to have been the editor. A plate showing Pope Sixtus V giving a sword to Hugo de Loubenx, and an emblematic plate specially engraved by Jacobus Bosius. It matches this description perfectly it seems.
